Good Faith Estimates - Patients

For patients who are uninsured, or who choose not to file a claim with their insurance carrier, we recommend that you contact your surgeon or the facility where you are scheduling your procedure. They are required to provide you with a quote for the total cost of your procedure, including the cost of professional anesthesia services.

If you would like an estimate for your professional anesthesia fees separately, our team is more than happy to provide that information to you. We will need the following information in order to provide you with a good faith estimate of our services:  

• Date of procedure 
• ICD Diagnosis Code – provided by your surgeon or facility.
• CPT procedure code(s) for your procedure provided by your surgeon/facility.
• Estimated Time that your procedure will take – provided by your surgeon/facility 
• Name of your Surgeon
• Facility where you will be having your procedure
